Does it seem like the likelihood of coming across quests that hand out medallions as prizes has decreased to almost none? Life has been very busy so I'm not able to quest every day (maybe once a week, twice a week at most), but I was able to at least get 1 medallion to use on RO a year. This was the only thing keeping my extremely poor SAs in the running, but now I'm not getting even a single medallion over multiple years.

Would also love to know this! I have done 505 quests since joining a couple of months ago and none of my completed/offered quests have rewarded one. I assume it's random which quests are offered, so appreciate that doing lots of quests doesn't matter... but it would be nice to know.

I actually have gotten 4 on my main account this game year and 1 or 2 on this one its completely random when you will get one for a quest

I've gotten quite a few even on my newest account. I'd say its just a stroke of bad luck to not get those quests
